A quick update on our diabetes management collaboration with Community Health Options of Maine.

Chronic care programs in health plans often sit at two extremes.

On one side are curriculum-driven programs like Omada, offering classes, videos, connected devices, and quizzes. On the other, minimal engagement: a few automated push messages or no structured support at all.

Both approaches have value but also limits. Academic-style programs can feel overwhelming and reach only a fraction of members. “Light” programs risk leaving members without meaningful guidance to manage their condition.

Inspired by the Buddhist principle of the Middle Path, Twig Health has partnered with Community Health Options of Maine to pioneer a more balanced, pragmatic model for diabetes care.

From Pilot to Full Program

Our work with Community Health Options began with a pilot focused on improving A1C testing compliance. Building on its success, this year we expanded to support Community Health Options’ entire diabetic population throughout the full care journey.

The AI-guided Front Desk Approach

Twig’s Front Desk model avoids the “homework” burden of legacy programs. Instead, members receive timely, personalized guidance through simple messaging and warm collaboration with nurse coaches.

We start by asking one fundamental question: “What are your biggest challenges?”

From there, Twig’s AI-powered framework guides nurse conversations toward practical, tailored interventions.

Some members choose deep engagement, actively shaping their care. Others prefer light-touch nudges at critical points, such as blood sugar management, nutrition, eye exams, and lifestyle adjustments.

Every conversation is powered by Twig’s AI platform, which adapts and guides our nurses over time so each member receives a personalized, evolving care plan.

Community Health Options’ Perspective

As Lori Tishler MD MPH, CMO of Community Health Options, explains “As a health plan, we have a vested interest in keeping our diabetic members healthy, and that requires the right amount of interventions at the right time. Twig’s Front Desk service expands our ability to engage members, providing longitudinal, granular, and personally engaging care that leads to better outcomes.”

Early Results

Since launch earlier this year, results are already encouraging:
– 36% of targeted members are engaged with the program
– ~30% of these members have activated various diabetic interventions

With a full-year  now underway, we expect to drive even greater engagement, activation, and impact.

A New Middle Path

Twig and Community Health Options of Maine are demonstrating that diabetes management does not have to be all-or-nothing. There is a middle path – personalized, scalable, and deeply human.
