Twig and SUNY Downstate case study – 940% increase in Healthfirst VBC performance

Since 2025, SUNY Downstate is partnering with Healthfirst and Twig Health to close gaps in value-based care and post-discharge member engagement.

Using Twig’s Front Desk model, we delivered 3,000+ VBC interventions, a 940% increase in VBC program performance, and a 1-star upgrade on both Medicaid and Medicare star ratings within a year, the best result in the hospital’s history.
